For a while in dentistry, it was all the rage to schedule blocks of time (usually 1 hours each) for the initial new patient exam with the doctor doing the bulk of the appointment themselves.

?To make this idea even worse, it was many times suggested that the doctor have the patient come back for a one on one sit down meeting with the patient in their private office to go over their findings.? Then, the doctor would report in miniscule detail all the stuff they found wrong with the patient and try to shove a HUGE treatment plan down their throats.? I sat through many lectures and seminars where this was the way treatment planning was approached.? It was even insinuated that if you didn't do things that way, you weren't much of a dentist.

I believe the term those presenters liked to throw around for anyone straying from their ideal was "mouth mechanics."? Hey, if you've got plenty of money, know how to do big case dentistry, and just like doing exam data capture and big presentations to patients as a hobby, then, by all means go for it.? Present away.? Heck, take a course on video and power point and really spruce up that case presentation.? But, for those of us living on planet Earth with a need to keep the new patient in the walls of our practice without running them off, and who need a case presentation acceptance percentage higher than the Mendoza Line (that's 20% for those non-baseball geeks out there), I think there is a better way.?

That way very much includes the doctor, but the doctor is used sparingly and in the role that only he or she is truly needed.? In Mississippi, I have to do the initial exam before I can delegate the x-rays.? That's a bummer.? I don't really see why I need to diagnose x-rays in this day and time of digital radiographs, but that's the way it is.? It may be that way in your state, too.? I don't know and I'm not a lawyer.? You'd better check on that yourself, but here's how I handle the exam.?

First, the new patient comes into the waiting room.? They are greeted by our new patient liaison staff member.? They are given a physical gift and their paper work to fill out.? Then, after the paper work is done, they are given a tour of the office patient areas by that same new patient liaison.? They are shown a couple of offices, a couple of operatories, our sterilization area, and our Studio room.?

This one operatory is set up nicer than the others.? It has a different color, the nicest dental chair in the office, more elaborate trim and molding than the other rooms, and better fixtures and pictures on the walls.? It is also set up for filming.? The patient is told that this is where Dr. Griffin does his smile makeovers and films for his seminars where he teaches dentists from all over the country.? What we are trying to do here is make it very obvious that we are not just another dental office.? This can be done by virtually anyone, anywhere.? Just be smart about it and don't overwhelm the patient to the point that they think they are in the wrong place.? Then the new patient is led to an operatory to be seated.? I have to pop in and do a cursory exam due to state law, so that's what I do.? I find it a waste of a perfectly good set of gloves, but I make the most of it.? When I first come in the door of the room, the assistant does a proper introduction.? Next, I do the exam-light and the whole time I am making small talk, trying to make a connection to the patient.? It is rare that I can't find something in common with them, heck; down here I am related to many of the patients distantly even if neither of us knew it before that day.? Then I order the x-rays and leave the room.? Wherever I am in the office after the x-rays are finished, the assistant comes and pulls them up for me so I can get an idea of what we are looking at.? We have digital x-rays or this strategy wouldn't work nearly as well.?

If it looks at all like perio, I'll probably order a probing by a hygienist.? Otherwise, it just goes up on the Route Board in sequence for me to come in for the final diagnosis.? When I can get back in there, I make a little more small talk as I am getting gloved up, then I do a complete exam from tooth #1 to tooth #32, just like in dental school.? I talk over the patient using common terms, but also adding in specific terms so the assistant knows what to write down for the treatment plan.? Once I am through with tooth #32, I make a little more small talk and maybe talk a little about a specific dental problem the patient is having.? By the time the assistant has entered all the procedures needed into our software, I have a pretty good idea of what the patient wants and expects and I also know what I am going to recommend.

Then I go over to the monitor next to the assistant to review the x-rays one more time against my findings.? Next, I like to look at the patient chart view of the software and I begin to group the procedures into appointments, placing the patient's chief complaint first if possible.? Next, I discuss what I think should be the first appointment with the patient while the dental assistant takes the Treatment Organizer card to the Financial Coordinator.? Once I have answered all the questions the patient has for me directly, I cordially exit the room telling the patient that the assistant will be back in shortly to go over the treatment in more detail.? Of course, I am not going over this whole thing in microscopic detail, but this is a pretty good overview of what happens on a regular basis.? Once I have left the room, I go back to the Route Board and get back into the mix of the day's procedures.? I may never see that patient again if financial arrangements couldn't be made or I may see them later that day.? It is all up to my staff.? The cool thing is that all the concerns of the new patient appointment leave my mind the instant I leave the room.? I have completely delegated the worry to my staff.? That's another good thing.

Dental Care For Cats

Most pet owners can spot their pet's health issues right away. One health problem that may not be so easy to find is dental disease. Cats, just like humans, can have problems with their teeth. As cats age, most do have some form of periodontal disease. Only a vet can tell for certain, but there are things that you can look for when you suspect dental problems with your cat.

Prevention

Most vets would agree that dental problems can stem from other sorts of health issues and should not be ignored. Proper health check ups with the veterinarian and spot dental problems before they get out of hand, In addition, there are several steps you can take as a pet owner to help your cat have healthy teeth and gums.

To help your cat's teeth stay in top shape, there are several things you can do. First, make sure your cat sees the vet, sick or not, once a year for a health exam. The veterinarian will be able to spot dental problems during annual exams. Your vet will look for bad breath, tooth decay, and missing teeth. All of these can point to more serious issues.

Another way to help your kitty have healthy teeth is to brush your cat's teeth. Yes, there are special toothbrushes and paste that will help your kitty keep his or her teeth healthy. Daily brushing is recommended, but for most cats, twice a week will suffice. Most pet stores carry cat toothbrushes and soft rubber brushes that fit on your finger. If your cat is reluctant, try to do only a small portion of the mouth at first and then add more each day. If you have a kitten, start brushing his or her teeth very early to get used to the routine. There are specially made toothpastes that are available. Most cats enjoy the taste and will think of brushing as a treat.

One product that is available to help keep kitty's mouth healthy is specially made rinses. These are good for older cats and cats that already have dental issues. Just as dogs can benefit from chewing so can cats. There are numerous products available for cats to chew to help their teeth and gums. Some are flavored with fish or poultry, which appeals to most cats. The abrasive texture of these dental chews helps remove food, debris and plaque that can build up on your cat's teeth. Extreme caution should be used when giving your cat a bone to chew for this purpose. Poultry bones are extremely sharp and brittle. Small pieces of bone can become lodged in your cat's throat or digestive tract causing serious trouble. Most vets recommend that chicken and turkey bones never be given to cats.

Major Problems

Proper dental care in your cat is very important. Problems with the teeth can be caused by or can cause other health problems including periodontal disease, feline stomatitis, Feline Odontoclastic Oral Resorption Lesions (FORL) and malocclusion. Most of these dental problems are treatable with proper medical care.

Periodontal disease is caused by build up of plaque and tartar. When tartar and plaque forms on the teeth is forces bacteria and food debris underneath the gum line and around the jawbone. This can cause inflamed gums and swelling and bleeding gums. Bad breath is another common sign of periodontal disease. This is the most common type of dental disease in cats and can be treated with a cleaning along with the use of antibiotics. Sometimes, extraction of some teeth is needed for extreme cases.

FORL is another form of dental disease often found in cats. This can be compared to a cavity in a human. Lesions on the teeth begin to form in the enamel of a tooth. When the plaque accumulates, the tissue around the affected tooth becomes inflamed. Sometimes the irritation can extend into the pulp cavity and can kill the tooth. If your cat has FORL, the vet will usually recommend extracting the affected tooth.

Feline Stomatitis is a very serious form of dental disease in cats. This is usually seen as an autoimmune disease and is most commonly found in cats that already suffer from more serious health issues. Stomatitis is caused when the body becomes allergic to the plaque on the teeth. Sometimes it can spread into the throat causing painful red lesions. When a cat has stomatitis, it will become very uncomfortable and may stop eating. You should seek the advice of your veterinarian right away.

Another dental problem is malocclusion -- when the teeth are not aligned properly. With a human, braces are in order. With cats, severe forms can cause eating problems and lead to severe weight loss.

Along With the many other advances in technology, you can now find specialized vets that deal with orthodontics. So maybe your tiger will someday be wearing braces while the other cats laugh behind his back.

Finding the Best Dental Training

If you are considering dental training to become a qualified dental nurse, or already working within the profession in another capacity but considering dental CPD, you need to understand the recognized routes into this role. It's vital that dental courses provide a solid foundation for your career progression, but it isn't always straight forward to understand the differences between them. This guide gives key points of the National Certificate and the NVQ level 3 Advanced Apprenticeship, and funding for dental CPD.

1. The National Certificate

This is dental training provided by the National Examining Board for Dental Nurses. Students must complete their Record of Experience, including Personal Experience Record Sheets (PERS) for numerous dental treatments and duties. There is also a 1,000 word case study to complete, focusing on one particular patient for a restorative procedure, a surgical procedure or the provision of a fixed or removable prosthesis. All this must be completed before taking the exam. This is then a 2 hour written paper, a spotter exercise, a practical exam and an Oral exam., All components must be completed successfully for qualification.

Of the two dental courses discussed here, it doesn't place much emphasis on English language, for example, spelling, punctuation and grammar. Sometimes students are at a disadvantage when it comes to the everyday operations within real life dental clinics. Letters have to be written, mental arithmetic helps when you're calculating billing and accounts and so on. You could consider whether a combination of The National Certificate and some further dental CPD such as Customer Service to NVQ level 2 or 3 and/or Business Administration could help you progress.

2. NVQ level 3 Advanced Apprenticeship

This is dental training administrated by the government recognized awarding body City and Guilds. Students must achieve level 2 of the through the "Key Skills" programme in both English and Maths through the "Key Skills", which is equivalent to GCSE Grade C. Communication Key Skills mean students are expected to be speaking confidently for over four minutes, develop competent discussion skills, be able to read and summarise documents in English well, and of course, write the appropriate documents themselves. The Application of Number component of the course may still not be all some students require for real life practice. Again, consider whether additional dental CPD in Customer Service NVQ level 2 or 3 and/or Business Administration will enhance your career prospects.

Bear in mind that over the next year, "Key Skills" are changing to "Functional Skills", whereby the English and Maths components will become part of a project which it to be directly relevant for dental courses by focusing on the dental workplace. In the future, these new "Functional Skills" components will be available as stand alone qualifications.

3. Funding Options

Whilst the majority of dentists recognize the much older National Certificate, funding isn't the same for these dental courses by any means. The National Certificate is accredited by the GDC and to attract funding, it must also be demonstrably structured and managed according to the standards stipulated by the Joint Qualifications Committee (JCQ). There is some NHS funding available, providing the dentist applying works under an NHS contract. By contrast, funding is typically more straightforward for the NVQ level 3 Advanced Apprenticeship, as it is run by a government recognized awarding body, namely the City and Guilds.

What most people don't know!

What you probably don't know is you can get the exact same services for around $43 per visit, or an annual cost of just $86. That's a huge saving of $178, or put another way 67%! (All figures based on national averages)
