Teeth whitening: "what is everything?"

What is it all about?

Teeth whitening is all about changing of the colour of the teeth to a lighter version of what you already have.

All materials whitening use to peroxide based material, be it Carbamide Peroxide or Hydrogen Peroxide of differing concentrations. For professionally applied materials, the concentration ranges from 3-30 per cent Hydrogen Peroxide and 10-28 per cent Carbamide Peroxide. Hydrogen Peroxide is about 3 times as strong as Carbamide Peroxide. The peroxide materials break down long chain molecules into shorter chain molecules, which changes the optical appearance of the molecules, therefore creating a lighter colour.

The size of the molecule peroxide materials is small enough that it can pass through the tooth structure into the pulp (nerve and vascular supply of the teeth).

How is it done?

There are two main systems in use:

1 In Chair / In Surgery Whitening BriteSmile e.g, Zoom, Quick Smile, Laser Whitening

2. At Home Tooth Whitening e.g Pola Day, Pola Night, Opalesence, Night White.

In Chair / In Surgery teeth whitening

These systems rely on using the stronger concentration ranges of the materials, accelerating the activity by using heat or cool blue light. The trouble with the heat producing systems (those using a heat light) is that they have the ability to harm pulpal the tooth tissue. Pulpal the tooth tissue should not be heated as it can cause long term side effects such as nerve damage, which can lead to other unnecessary treatment. This author prefers to use the BriteSmile "in chair" system. The BriteSmile system uses the cool blue light, drastically reducing the possibility of long term side effects.

At Home teeth whitening

The most common version of this system is when the Dental Practitioner makes some thin, mouthguard like trays for the patient. These trays are filled with a whitening gel and the patient wears the trays daily over a period of 2-3 weeks, until the desired effects are reached.

Will the Results Differ Between the Two Systems?

The short answer to this question is no. If the patient is compliant and carries out the necessary number of applications with the "at home" system, usually 10-14, then the results are the same as the "in chair" systems. The main problem with "at home" teeth whitening is patient compliance, carrying out the necessary applications of the material. Generally speaking, most patient s are diligent for 3-4 days, therefore never really achieving the great results that "at home" teeth whitening can achieve.

This is why the "in chair" systems are often better to use. It removes the biggest problem with "at home" teeth whitening, which is patient compliance. The different systems have vastly different levels of the short term transient side effects that come with teeth whitening.

The ideal scenario is if patient s are encouraged to use both systems. This means that the patient undergoes to session of "in chair" whitening so that the teeth change colour within a limited time frame, but then an "at home" kit is also used for 2-3 days following this session to stabilise the colour. The reason for this is that up to 40% of colour change from the "in chair" system is dehydration of the teeth. Thus, as the teeth rehydrate, the colour fades. The "at home" teeth whitening helps to maintain the initial colour change, but also allows the patient to touch up the colour over the long term, with little expense. It is important to enable the patient to touch up the colour because the colour will fade over 2-3 years.

What are the most common side effects?

The most common side effects are;

1. Tooth Sensitivity;

2. Gum Soreness;

3. Sore Throat;

4 It nausea; and/or

5 From-mineralisation of the enamel (outside tooth structure)

The most common side effects are tooth sensitivity and gum soreness. The literature has revealed that some people do get a sore throat and a little nausea, but really only from the "at home" system, and only rarely.

The tooth sensitivity from the materials is passing into the pulpal tissue and causing an inflammatory response. The gum soreness can be from the peroxide materials irritating the tissues or, interestingly, it can be caused by trauma from the egdes of the trays pressing into the gums, and not the whitening materials at all.

In the literature, the of-mineralising of the tooth tissue has been likened to drinking a can of coke. This is easily and quickly reversed by the saliva. This is a normal, everyday event that happens in the mouth. Teeth whitening is only an issue in this regards if carried out all the time, over many years. Once again, the damage can be reversed and controlled with proper instruction.

The side effects generally subside within 7 days, after the whitening has been completed

Conclusion

teeth whitening is highly successful and safe to use under the proper supervision of a dental professional. The most successful protocol to follow for overall good colour change and high patient satisfaction, is to combination of both the "in chair" and "at home" teeth whitening systems. This allows the patient to start from a very good colour change position and then take control of stabilising and touching up the colour at their own discretion, cheaply. This increases long term patient satisfaction. There are some transient short term side effects. It is therefore necessary to be under the care of to trained dental professional when undergoing teeth whitening.

Facts about Home teeth whitening

Today, there are many teeth whitening kits that you can do at home. But you might get shocked, if I tell you that whitening toothpaste works only as a metalbehandling, not as a whitener. It can get rid of a few spots, but cannot give them shiny teeth.

A high concentration of carbamide peroxide or its hydrogen peroxide can be found in the teeth bleaching kits. Hydrogen peroxide is the most effective substance compared with the other one. 5% concentration of hydrogen peroxide, is equal to 15% carbamide Peroxide concentration. But both substances can irritate your teeth and gums. You should consult your dentist to get a low concentration at-home kit.

You must leave the lower-concentrated bleaching agents at the teeth for a longer period of time, and this is actually the more affective way. Kit from a dentist contains a mouthpiece, which fits to your teeth, so you can easily place your whitening gel, which comes in a pre-measured container.

These whitening kits can be purchased via the Internet or on the "drugstore. To cost a lot less, but it is not the costumed be adapted to your teeth. You can get for less than $ 20 to disposable mouthpiece and it costs the same for the reusable. But you should still get carbamide gels are to costing about $ 10.

Apply a 35% carbamide concentration, you can leave it in your mouth for as long as 30 minutes up to two hours. If you use the low concentration of hydrogen peroxide solution, you can leave it overnight.

The other two settings also whitening strips and paints on bleaches. Strip hurt less, and you can use it for a small spot. But paint on bleaches are not recommended for the inefficient, and it has the same price as the gel provided by the dentist. You should get a cheaper, if you want to try this, an expensive will have the same results as the cheaper.

Please remember that any kind of teeth whitening kit cannot bleach fills, crowns, bridges and veneers. If you keep on trying, you want to have a darker tooth, which will be seen easily, but you still van replace your veneer with it that corresponds to your whiter teeth.

Most dentists use the concentration of hydrogen peroxide as much as 40%. As you can see, this is much more, compared with 35% carbamide Peroxide.

The effectiveness of the Office processing could be the same with teeth whitening kits home provided by a dental office. They may have a lower concentration of hydrogen peroxide, but they are typically left on for a longer period of time. Dentist treatment takes time, as long as 15 minutes Home kits can be left the teeth in the night, which can lead to higher point.

Ask the dentist: the truth about Lumineers

Q: I'm 47 years old, and I had made of Lumineers about six years ago. They were all good, but they were not as natural looking as I had hoped and the form was not quite right. I am prepared to replace them with new veneer. How do I tell my dentist what I want?

A: Let me tell you why Lumineers are a good product. They have relatively cheap in comparison with traditional veneer. Little to no sanding is done in preparation for the teeth of the porcelain, and finally, it is not a proceeding relating to a high of skill or expertise of the dentist mate.

In short, the results are fairly predictable. It is easy to get whiter and straighter teeth with Lumineers.

But, as you have seen, the results are rarely spectacular or natural looking.

When an expert porcelain ceramist a porcelain veneers, be he a perfectly prepared tooth for a predictably spectacular result.

You can imagine that the same is true for a furniture maker, that is a master craftsman. If the floor level and the walls need your furniture maker square are to "fudge" this and make up for that.

Tooth preparation makes a difference. When the teeth have to be prepared, Lumineers are fine, but I think can those types of cosmetic cases are relatively rare and replace existing veneers bring his own challenges.

Your communication with your dentist will be critical to getting the look you want. How long or short, how round or square, how white or not?

But you do not do this for a living, your dentist does. It is the task of your dentist to interpret your desires and dislikes and come up with a solution.

I've found, more than 25 years, that this is impossible without an intermediate step. Your dentist should be able to give you a nice set of temporary veneers that meet your expectations or allow for a discussion of any amendments which are necessary, where appropriate, before the permanent porcelain is manufactured.

Your role in the process is a little simpler. The collection of photos of the smile that you want and bring them to your dentist. The photos will serve as a basis for discussion and you and your dentist can decide what is important to you.

Not everything you may want to, but you must be told before you initiate the process. Your expectations must be reasonable.

Two different patients may have the same teeth, but the bone who holds the teeth in the shape of the gums, shape and fullness of their lips and their complexion can all change the final result.

Take your time and enjoy the process. By the time you are ready to start, you and your dentist already have a pretty good idea of the final results.
